Social Care - Adult Social Care Transitions Team

The Adult Social Care Transitions Team supports young people with an Education, Health and Care Plan (EHCP) as they move from children's services into adult services.

The team works across Wakefield and is part of the Community Team for Learning Disabilities (CTLD East).

How the service supports

The team works with young people from around age 16 (or earlier is needed) until they leave full-time education. 

They complete a strengths-based assessment to:

  • Understand the young person's needs
  • Identify their goals and aspirations for adult life
  • Determine whether they are likely to meet the criteria for social care support under the Care Act

The assessment explores what support may be helpful, including both formal services and community-based options. 

Planning for adulthood

The young person is at the centre of all decisions. The assessment considers:

  • Their views, wishes and feelings
  • Their goals for independence and adult life
  • The support needed to achieve these outcomes

Advice and information are provided to both the young person and their family about services and opportunities available locally. 

Assessments are usually completed around age 17, but can take place earlier or later depending on individual circumstances. 

Support and funding

Young people who are eligible for adult social care may receive an individual budget to support their needs. This can be managed in different ways, including:

  • Direct payments
  • Individual service funds
  • Managed accounts
  • Personal health budgets, where appropriate

Supporting families

The team works with parents and carers throughout the transition process, offering guidance and support to help them in ther caring role and plan for the future.
