Health - CAMHS - Responsive Adolescent and Children's Home-based treatment Team (ReACH)

The ReACH Team provides urgent and responsive mental health support for children and young people in the community or within a hospital setting. 

The service is designed to offer quick assessment and short-term support during a mental health crisis or when additional support is needed. 

How the service supports

The ReACH Team offers:

  • Emergency and urgent mental health assessments
  • Up to seven follow-up appointments in the community after attendance at services such as Pontefract Urgent Treatment Centre, Pinderfields Hospital, or other hospitals (for those registered with a Wakefield GP)
  • Intensive home-based support to help prevent hospital admission or further decline in mental health

The team also supports children and young people when they are leaving hospital or in-patient services, helping to ensure a safe and supported transition back into the community.
