How do I get on the Dynamic Support Register?

Speak to a professional involved in your care about the DSR. They can make a referral on your behalf. You can also self-refer onto the DSR by completing the SWYT DSR Self-Referral Form and returning it to the email address provided on the form.  

The SWYT DSR General Consent Form should be completed to confirm that you understand what the DSR is and agree to your information being shared with the professionals involved in your care and support. If the person being referred does not have capacity to provide informed consent, a best interest decision must be made. For children and young people, a person with parental responsibility can provide consent on their behalf.
