We provide palliative care, and personal care to people with dementia and physical disabilities. We also support people with various forms of learning disabilities.

Our mission is to identify service users’ needs, develop person-centred care plans and deliver very reliable services that improve the wellbeing of services users, working together with family members and multi-disciplinary teams.

At Croft Healthcare, we support all age groups.
We require an Educational Health Care Plan for us to understand the care needs of an individual.
We currently do not have a waiting list in place.
We work with referrals that involve professionals
