An education, health and care (EHC) plan is a statutory legal document for children and young people between the ages of 0 to 25 who need more support than is available or deliverable through special educational needs (SEN) support.

EHC plans identify special educational needs and associated health and social care needs relating to SEN and set out the additional support (provision) required to meet those needs.

The purpose of an EHC plan is to make special educational provision to meet the special educational needs of the child or young person, to secure the best possible outcomes for them across education, health and social care and, as they get older, prepare them for adulthood.
